- My students signed a parental consent form before they started the project. I can provide you with copies, if you need. Those forms include all of the information in your form. In addition, the students completed the survey. I did not have them put their names on it. I can aggregate the results and snd them to you, if you need. Also, when you say 'visit,' do you mean in person or via web cam (just curious)?
